Please print and fill out the forms below. You may email them back to info@metrodog.com, fax them to us at 510.526.1367. Once we have received your completed enrollment forms we will schedule your intake assessment appointment.

Reservations – For Daycare and Boarding

 If you and your dog(s) are an EXISTING Metro Dog clients e-mail info@metrodog.com,us with the following information:
Your Name
Your Dog’s Name(s)
The Dates you need care
Daycare or Boarding
Type of Room or Program
Pick up time on last day
Check out is 10AM – no charge
Half day of Daycare before 3PM
Full day of Daycare until 9PM
Any Additional Services: Medications, Leashed Walks, Park Trips, Tune-up Training, Baths, Nail Trims
and let us know about anything else that will help us provide great care for your dog.

New clients need to call or email for an orientation appointment.

Important Reminders

  • Food must be packed in a flat bottomed container that seals – NO BAGS.
  • Please label all food containers, cans and bedding with your dog’s name.
  • All dogs must be current on Vaccines for Rabies, DHP, and Bordetella.
  • If it has been a while since your dog was last with us we recommend bringing them in for a day of daycare to get re-acquainted with the routine. Sometimes we will require this before confirming a boarding reservation.
  • Please note that  filling out this form is not a guarantee of services. We will confirm your reservation by email – If you haven’t heard back please send another email. Sometimes emails just find their way to the land of digital lost socks.
  • Thanksgiving, Christmas, and New Year Holiday Boards have special requirements. Please see our Deposit and Cancellation Policy.
